DD Form 2875: understanding a System Authorization Access Request
Identify the official SAAR form and confirm the receiving system’s access process before preparing a request.

Start with the official form
The Department of Defense lists DD Form 2875 as the System Authorization Access Request (SAAR). Use the official forms directory and the receiving organization’s instructions to identify the applicable edition and process.
Confirm the request before preparing it
Ask the responsible system-access team which form, supporting information and submission channel apply. Do not substitute a generic checklist for their instructions or assume one organization’s approval transfers to another system.
Keep the request and the decision separate
Preparing a request does not grant access. Keep the requested scope, the responsible reviewer and the actual authorization decision distinct. Follow the receiving organization’s handling requirements for personal and system information.
